Why interpret TSH, Free T4 and anti-TPO together?
Each number alone can mislead. A raised TSH means something very different with a low Free T4 (overt hypothyroidism) than with a normal Free T4 (subclinical), and positive anti-TPO antibodies change whether borderline results are treated. The pattern across all three is what guides treatment.
What is subclinical hypothyroidism?
A raised TSH with a normal Free T4. With positive anti-TPO antibodies the risk of progressing to overt hypothyroidism is high and many endocrinologists treat; with negative antibodies it may resolve and is often rechecked in 3–6 months.
